Request for Non-Release

Wisconsin Open Records Law

Records collected and maintained by the Department of Public Instruction, including data contained on the Educator Licensing Database, are subject to Wisconsin's open records law, Wis. Stat. 19.35. The open records law requires public agencies to make its records available for public inspection unless a specific exception to the law applies. Periodically, the department receives open records requests for information about individuals who hold, have held, or have applied for a Wisconsin educator license. As these are public records, the department is required by law to release educator license database information unless an exception under the law allows the information to be withheld.

Request for Non-Release

The only exception under the law which prevents the release of license database information is if the release of personally identifiable information could endanger an individual's life or safety. DPI license holders may request a non-release of their home address or home phone number that are in the educator licensing database.

Please use the following form to request for non-release of your information:

  • PI-1662-A: Non-Release of Personally Identifiable Data

This form must be completed and signed in the presence of a notary public. Mail the original to DPI Madison address. Upon receipt of a properly completed form, the request will be processed and the requestor will be notified in writing upon completion of processing.
