School Occupational Therapist, Occupational Therapy Assistant, Physical Therapist, Physical Therapy Assistant

We will continue to work with educators who had reached out for assistance or started an application online prior to June 30th to be able to assist with technical issues that prevented application completion.  Applications from these educators who made a good faith effort to apply will be considered as having been made timely. ******

Any person employed by a school district as a school occupational therapist (OT) or occupational therapy assistant (OTA), or a school physical therapist (PT) or physical therapy assistant (PTA) must hold related license issued by the Wisconsin Department of Public Instruction.

License Application Requirements

To apply for a School Occupational Therapist or Occupational Therapy Assistant, School Physical Therapist or Physical Therapy Assistant:

  • Applicant must hold a valid license issued by the Wisconsin Department of Safety and Professional Services (DSPS). Include the DSPS license number and/or a copy of the DSPS wallet card with the application.

License Renewal Requirements

To renew a School Occupational Therapist or Occupational Therapy Assistant, School Physical Therapist or Physical Therapy Assistant:

  • Applicant must hold a valid license issued by the Wisconsin Department of Safety and Professional Services (DSPS). Include the DSPS license number and/or a copy of the DSPS wallet card with the application.
