School Occupational Therapist, Occupational Therapy Assistant, Physical Therapist, Physical Therapy Assistant

Any person employed by a school district as a school occupational therapist (OT) or occupational therapy assistant (OTA), or a school physical therapist (PT) or physical therapy assistant (PTA) must hold related license issued by the Wisconsin Department of Public Instruction.

License Application Requirements

To apply for a School Occupational Therapist or Occupational Therapy Assistant, School Physical Therapist or Physical Therapy Assistant:

  • Applicant must hold a valid license issued by the Wisconsin Department of Safety and Professional Services (DSPS). Please upload a copy of the DSPS license or wallet card with the application.

License Renewal Requirements

To renew a School Occupational Therapist or Occupational Therapy Assistant, School Physical Therapist or Physical Therapy Assistant:

  • Applicant must hold a valid license issued by the Wisconsin Department of Safety and Professional Services (DSPS). Please upload a copy of the DSPS license or wallet card with the application.
