ELO First-time Application Tips for OT/PT/OTA/PTA

****** We have experienced an overwhelming volume of phone calls and email these past few weeks seeking assistance with ELO and licensing. We will continue to work with educators who had reached out for assistance or started an application online prior to June 30th to be able to assist with technical issues that prevented application completion.  Applications from these educators who made a good faith effort to apply will be considered as having been made timely. ******

Directions for first-time Occupational Therapist, Physical Therapist, Occupational Therapist Assistant, and Physical Therapist Assistant applicants

IF there is some type of misconduct that needs to be reported, prepare and attach a written explanation of what happenedYou must also provide complete electronic (scanned) copies of documents associated with the matter that can corroborate / illuminate your explanation of the incident. These may include the criminal complaint, judgment of conviction, police reports, disciplinary letters/findings, correspondence, etc., as applicable.  (Note: Court history information printed from the CCAP web site is NOT sufficient.)

  • Scan your valid WI Department of Safety and Professional Services License into a PDF file that is less than 2 MB in size and save the file so that it is readily available to upload into ELO.
     
  • Have your credit card ready.  If you do not already have a credit card, purchase a pre-paid credit card at a convenience store.
  • Log into the ELO system - http://elo.wieducatorlicensing.org to apply. Find the section toward the bottom of the window for starting a new license application.  Make the appropriate choices similar to the ones below, then click Select and follow the directions.
